  • "A" Format Scoresheet
    Official scoresheet for Tuesday/Thursday A-Division (A1-A2) Team Leagues.

  • New Player Registration Form
    When issued by the League, this form will have a date in the lower left corner. All player information is required and the form must be signed by BOTH the player and the Captain. The form MUST be returned within two weeks of the new player's addition to the team,
  • Team Signup Sheet
    This is a generic signup sheet without dates; the forms distributed by the league are different each season so that dates are included. This form may be used instead but you are responsible for remembering the dates and deadlines. Information for players 5 - 8 are now entered on page 2.
    UPDATED: Sponsor fees were raised in January 2007 and this document was updated accordingly. Until it can be converted to a "pdf" file, it is a Microsoft Word 97-2002 document.
  • Candidate Letter of Intent
    This is the same form as distributed by the league that is used to run for office. This form (or a hand-written version) must be submitted to the league Secretary at least 10 days prior to the first election night (this usually falls on a Friday since general elections begin on Monday nights). Note: Candidates for PRESIDENT must also submit a petition (see below).
  • Petition Form for Candidates for President
    This is the same form as distributed by the league that is used to run for league President. Verifiable signatures from 30% of the membership (or 100 members, whichever is less) must be submitted to the league Secretary at least 10 days prior to the first election night (this usually falls on a Friday since general elections begin on Monday nights).

    As modified by the Board of Directors on February 11, 2001.
